Job Description

Position Summary: We are looking for a passionate Intake Coordinator who will be responsible for coordinating the intake and admission of patients. The Intake Coordinator will perform pre-admission functions to determine resource allocation of potential clients, including performing clinical assessments. The Intake Coordinator will respond to referrals and coordinates the admission of individual patients into the inpatient, outpatient or partial hospitalization programs, complete authorizations and referrals.

Responsibilities

  • Managing and performing timely intake processes on referrals including completion of the referral page and family details page if applicable
  • Aiding in tracking enrollment and supervising timelines by considering federal guidelines, completing various reports and forms, drafting mails and letters as well as resource materials to the families of the patients
  • Creating and maintaining database for well-organized assortment and retrieval of patients and line up details for outside agencies and internal staff
  • Participating in team meetings, handling correspondence, sharing meeting responsibilities, responding to routine requests, and assisting with the collaboration of professional learning and professional development
  • Schedule New Patient appointments and explaining New Patient Policies in a clear and professional manner.
  • Start, complete and monitor authorization for the OMH and PRP clients.
  • Schedule Internal Referrals and following-up with the referring sources.
  • Maintain open communication with the Operations Manager & Clinical Director.
  • Assist client in accessing forms as well as tracking intake forms for prospective patients, in charge of Intake email for correspondence from providers/patients.
  • Process all incoming intakes – gathering additional information as needed,
  • Do walk-in intakes as needed- front desk knows to inform intake when walks ins occur- check with front desk on room availability.
  • Responsible to maintain provider grid up to date; have working knowledge of all services, providers, and staff.
  • Orient client about healthcare services available
  • Assist clients and clinical staff with transferring between providers as needed.
  • Responsible for keeping records of internal referrals and transfers.
  • Verification of insurance for clients/patients as needed
  • Perform clerical duties as needed
  • Educate clients on Tebra set-up and completion of forms.

Skills

Proficient in using computers for data entry; successful candidates must:

  • Have knowledge of Microsoft Excel, Word, Adobe and experience of working with database management.
  • Familiar with essential medical procedures and terms since they work closely with the patients
  • Possess a level of patience and, a pleasant demeanor essentially with a high level of problem solving skills.
  • Aware of federal indicators, local and state procedures in addition to flexible and detailed oriented is required.

Education/Qualification

  • Bachelor's degree or higher in social work, psychology, human services or in any related mental health field is required
  • At least one (1) to two (2) years of experience working in this field is required.
  • CPR and First Aid required.
  • Must have a valid driver's license and vehicle to conduct home visits.
  • Must have excellent organization skills and have excellent case note documentation skills.

Job Type: Full-time

Salary: ₦250,000.00 - ₦300,000.00 per month
